Step 1: Initial Contact
A phone call to our office. One of our care specialists will discuss your needs with you, explain options for different types of care and means of accessing it, and if home care is an applicable option, arrange for an on-site assessment.
Step 2: Consultation Assessment
A FREE in-home assessment appointment with one of our nursing professionals is set up to meet with the patient (and, where appropriate, their family members) to discuss the care required. During this meeting, all aspects of care are discussed (environment, client health conditions, health needs and goals). From this meeting, if care is requested, specific care plans designed for your particular needs and goals are developed.
Step 3: CareGivers Contract and Consent Form
This agreement is signed only after the client and the other involved parties thoroughly understand the rights and responsibilities under the agreement, including schedules, services, and invoicing.
Step 4: Delivery of Care
Once engaged, CareGivers will begin the process of care delivery, which includes:
- Selection and scheduling of suitable staff, based on personalities, skill and experience, availability, and preferences
- Orientation of staff to the clients envrionment and care plan
- Delivery of services according to the plan developed
Throughout our service delivery, CareGivers' quality control programme will help monitor the quality of care you receive. A combination of periodic site visits from our nursing supervisors, and solicited feedback (both formal and informal) will help us know if we are best meeting your needs - and where we are not, changes and adjustments to your care plan can be made.
